If you are a new client to MindBody Massage, you will be asked to fill out a client intake form, for us to keep on file. In order to save time and/or having to show up 15 minutes prior to your first appointment, please feel free to print out the "First-Time Client Health History Form" and fill it out to bring with your to your appointment.

On Occassion there may be special health related reasons (whether temporary or chronic) where a doctors permission will be requested by MindBody Massage, before we can preform a massage. In this event, please print out the "Physician's Permission Form", by clicking on the link below and bring to your scheduled appointment.
